1) Once your approved and receive your financing limit, select the 'Complete Loan Application' button to move forward.

2) From there, select 'Connect Debit Card' to connect your personal debit card.

3) Once the card information is entered, select 'Submit' to continue. 

4) On the following screen, you'll be able to review the card you have entered. To proceed, select 'Continue.'

5) Select 'Accept' to accept the loan invitation from your dental provider.

6) On the next screen, you'll be asked whether you are ready for pay for treatment. 

Note: If you know the final treatment amount, select 'Yes' to proceed with taking out the loan. If you do not have the final treatment amount, please select 'No' until you have the final treatment amount from your dental provider. 

7) Next, enter the amount you wish to borrow and select 'Continue'

8) Then select your repayment schedule and select 'Continue'. Typically, customers select when they get paid to match their repayment frequency. 

9) Select how you would like to repay and select 'Continue.' For seamless repayments, we highly recommend selecting the 'Autopay with Debit Card' to avoid any late payments/fees.

10) Finally, select your repayment plan and click the checkbox next to 'I agree to the repayment terms mentioned in the TILA Disclosure.' Then hit 'Submit' to finish taking out your loan.

And that's it! Your Basis loan has been taken out. You can see a summary of your loan here.
